Care Coordinator RMN Opportunity available for a Registered Mental Health Nurse to work as a care coordinator, Based in Wallingford in Oxford The team sits within Older Adults Community Mental Health Team based in Wallingford with the role predominately providing care to older adults with mental health needs both functional and dementia. Care Coordinator RMN Opportunity available for a Registered Mental Health Nurse to work as a care coordinator, Based in Wallingford in Oxford The team sits within Older Adults Community Mental Health Team based in Wallingford with the role predominately providing care to older adults with mental health needs both functional and dementia. You will carry out backssments of mental health needs, care planning with psychosocial interventions and medication. As part of this role you will be out in the community providing care coordination so a valid driving license with access to a vehicle is essential. It is also essential you have experience working within the community and experience with older adult mental health is desirable but not essential. This role is full time working 37.5 hours - Monday to Friday, 9am to 5pm. There would be a possibility to work 4 longer days. Hourly rate for this role is: £28 Per Hour We are offer a £250 sign up bonus for any new nurses that register with our agency. This will be paid in your first pay packet. Please also bear in mind our £250 referral fee bonus for any nurse you refer who we place in to work on a 3 month contract.
