WHAT YOU'LL DO The Jewish Community Caseworker will promote client rights and access providing case management services specifically for members of the Western New York Jewish Community. They will work in a patient-centered and culturally sensitive manner with Jewish community members, regardless of Temple affiliation or non-affiliation and may also provide services to people whose significant other may not be Jewish to educate, enroll, and manage health care and social needs. Key Responsibilities Client Services Assist all members of the community and their families/caretakers to navigate the healthcare and social services system including home visits, financial management, access to public benefits and medical appointments. Case Management Prepare treatment plans, discharge plans, and follow-up care programs; provides therapeutic crisis intervention and emergency services as required followed by referral to appropriate resources. Collect data about clients through interview, case history, psychological tests, and/or observational techniques; evaluates data to identify causes of problems and to determine proper therapeutic approach or referral to other specialists. Referrals Collaborate with medical providers, social service providers, other patient services, and agencies to improve patient care and outcomes and facilitate cultural and linguistic access. Register clients for appointments with appropriate staff members. Administrative Access interpretation services to ensure clients receive services regardless of language barriers. Complete intake applications and needs assessments. Process paperwork on new clients into the electronic medical record in a timely manner and in accordance with protocol. Establish and maintain case files as needed. Perform clerical duties such as inputting data, mailing and filing, copying, creating, and updating lists and/or records. Handle sensitive agency-related information in a confidential manner and adhere to all HIPAA and other important guidelines/regulations. Advocacy Consult with other therapists and related professional and paraprofessional staff, as appropriate, in the performance of therapeutic and/or casework; refers clients to appropriate service agencies as required. WHAT YOU'LL BRING Minimum of a Bachelor's degree in Social Work (MSW Preferred), human services, or a related discipline required. Experience providing case management. Working knowledge of local community and holocaust survivors' resources. Demonstrated ability to maintain effective case notes and documentation. Ability to work in the office and community with flexibility to work evenings and weekends to attend community programs and activities is required. Excellent organizational and time management skills; ability to prioritize effectively. Strong computer and internet skills, particularly with Microsoft Office and Outlook. Excellent written and verbal communication skills. Ability to maintain a strict level of confidentiality. Provide exceptional level of client-centric service. This position requires travel between sites and therefore, you must have a reliable vehicle, valid driver's license, and liability insurance in the amount of $100/$300K. Also, if hired, you must produce evidence of having received the Covid-19 vaccination. WHAT WE OFFER Competitive salary range of $21.00 to $28.00 per hour, commensurate with experience and qualifications. Benefits such as health insurance (Independent Health), accrued Paid Time Off (PTO) of 4+ weeks, 401k retirement plan with company match, and 13+ observed holidays.
