The Opportunity Description We're looking for a RN, working in Healthcare Systems and Services industry in Minneapolis, Minnesota, United States. Our team of clinicians cared for more than 4,200 patients across 185 primary diagnoses. We combine biometric monitoring, digital surveillance, in-home nursing and therapy, virtual visits with hospitalists and geriatricians, and a comprehensive supply chain to deliver a safe and high-quality care program. Opportunities: Multiple full-time and part time RN positions available! Weekend and holiday rotation OR working on a casual/as needed basis. What you'll be doing as a RN: Complete nursing assessment, monitoring, evaluation, interventions, and treatment to care for acute care patient populations in the home. Collaborate with the patient, family, interdisciplinary team, therapists, specialists, and use community resources to provide effective care coordination; evaluate social determinants of health and incorporate into the patient's individualized care plan and care management. Assist with biometric monitoring technology and respond to biometric vital sign data. Provide individualized patient education on disease and symptom management, medication management, treatments, procedures, and therapies. Prepare for and coordinate discharge from hospital at home with patient and family to ensure seamless transition to patient's PCP, home care services, and specialists, with community resources as appropriate. Competence in procedural skills such as: phlebotomy, vascular access device management, hydration therapy, medication administration, wound management, catheter care, specimen collection, stoma management, and compression therapy. Participate in quality and safety investigations and root cause analyses; take appropriate action to ensure patient safety as a result of any investigation. Experience Required for Your Success What we're looking for: Associate's, Bachelor's, or Master's degree program in Nursing. BSN strongly preferred. One (1) to Two (2) years of RN experience in a healthcare setting. Ideal candidates would have at least 2 years of home care, acute inpatient care, or emergency department experience. Preferred experience with procedural skills listed above. Current RN state license required. Experience with technology including EMR systems and Windows-based computer skills. Can work independently with the patient in their home, be organized and resourceful, with strong communication and collaboration skills.
