These recognitions highlight LVHN's commitment to teamwork, compassion, and technology with an unrelenting focus on delivering the best health care possible every day.Whether you're considering your next career move or your first, you should consider Lehigh Valley Health Network.SummaryCoordinates the services for solid organ transplant patients and communicates with the multidisciplinary patient care team, nephrologists, and dialysis centers, staff, and other healthcare providers.Job DutiesDaily care of transplant patients in the outpatient clinic.Coordinates care and discharge of patients from the inpatient setting.Reviews medical history, coordinates schedules for medical testing, and reviews results to ensure optimal outcomes.Obtains and reviews referral information from referring and consulting providers and other referral sources.Ensures completion of appropriate testing and provides results to physicians and advanced providersParticipate in patient education and discharge planning in the pre and post operative settingFacilitates development and management of clinical protocolsParticipate in on call duties for the transplant program on a rotating basisCollaborates with the Organ Procurement Organization and on call surgeons to determine suitability of donor organs.Manages admission of potential donor and recipient to hospital for surgery.Minimum QualificationsBachelor's Degree in Nursing4 years Experience as a nurse in the medical, surgical, and or dialysis field.Demonstrated knowledge of current transplant-related medical information.American Heart Association Basic Life Support - State of Pennsylvania Upon HireRN - Licensed Registered Nurse_PA - State of Pennsylvania Upon HirePreferred QualificationsMaster's Degree in NursingPhysical DemandsLift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.Job Description Disclaimer: This position description provides the major duties/responsibilities, requirements and working conditions for the position.
