Job ID: 24-02097Social Worker - Licensed Graduate Professional CounselorWashington, DCMUST:The Licensed Clinical Social Worker or Licensed Graduate Professional Counselor must have experience:Master s degree in social work, psychology or relevant human services disciplineActive LGSW or LGPC license issued in the District of ColumbiaDemonstrates knowledge of mental health principles and demonstrated ability to operationalize and implement these principles in mental health practiceProficient in Microsoft Office products (Word, Excel, Outlook)Case Management experienceExperience documenting case notesAbility to identify and resolve problems in a timely manner even when dealing with emotional topicsDUTIES:Participate as team member in the development of a consumer's Individual Recovery Plan (IRP) under the direction of the clinical supervisorAssume responsibility for the implementation of the IRP in coordination with the team and under the supervision of the team supervisorDevelop therapeutic relationship with assigned clients to enhance functioningIntervene to assist consumers in acquiring skills and resources necessary for successful community livingProvide support and consultation to members of the consumers' networkAssist the consumer in symptom management, problem solving, and resource developmentParticipate in crisis prevention and intervention on behalf of the team s clients as neededEnsure that each consumer has an individualized relapse prevention plan that is shared with all appropriate providers and network membersParticipate in leadership of psycho-educational and skill development groups as appropriateParticipate in ACT Team daily morning meetingsDevelop strategies with the consumer so that they can maintain living outside of institutional settingsComplete diagnostic assessments and treatment plans under the supervision of the team supervisorAssume responsibility for special projects on the team and within the ACT programComplete all required paperwork on timeQuadrant is an affirmative action/equal opportunity employer.
