We are devoted to helping Maui's Elders live their best lives. Hale Makua's Kahului location is a 254-bed nursing facility. The Position: We're looking for a passionate Resident Assessment Coordinator who will complete and coordinate all aspects of the resident assessment and care planning process. Works in collaboration with the IDT to perform a comprehensive assessment based on standards of nursing practice, federal and state regulations related to the RAI. Oversees appropriate and accurate recordation of information on the Minimum Data Set. Works with the Neighborhood Supervisor and/or Charge Nurse to assure the plan of care is disseminated to all nursing staff. Educates/coaches other IDT members on the technical aspects of the RAI and the care planning process. Requirements: Graduate of an accredited school of nursing. Two (2) years as a Registered Nurse Experience in long term care preferred Registered Nurse with the State of Hawaii Board of Nursing. Current and valid 2 Step TB clearance and health review (can be done in-house).
