We are currently looking for an experienced PCN Care Coordinator, who can provide overall support for the clinical leadership of the service. This post is based in London, which is easily accessible via public transport. About the role: As a PCN Care Coordinator, you will play a key role in proactively identifying and working with people with long-term conditions, to provide coordination and navigation of care and support across health and care services. Your main responsibilities as a PCN Care Coordinator: - Proactively identify and work with people, such as the frail/elderly including those with long-term conditions, to provide coordination and navigation of care and support across health and care services - Work closely with GPs and practice teams, supporting them to understand and manage their condition and ensuring their changing needs are addressed - Provide support by working alongside social prescribing link workers to enable people navigate through the health and care system - Support the PCN management with conducting searches on data cohorts and helping to meet Impact and Investment Fund IIF indicators and Enhanced Service workstream targets What we are looking for in a PCN Care Coordinator: - The legal right to work in the UK - An NVQ Level 3 in adult care - advanced level or equivalent qualifications or working towards it - Experience of working in health, social care and other support roles in direct contact with people, families or carers in a paid or voluntary capacity - Proficient in MS Office and web-based services Why work with Maxxima - Knowledgeable, dedicated and vastly experienced Permanent Team with longstanding professional relationships with multiple clients.
