$2,500 Sign-On Bonus!!!! Come join a vibrant, interdisciplinary team serving the complex needs of adults with an opioid use disorder in the greater Portland area. We are looking for a collaborative, engaging, and driven care coordinator (case manager) who has their MHRT/C or is MHRT/C eligible. Care coordinators work in partnership with the Opioid Health Home Treatment team to help individuals meet their substance use and mental health recovery goals. This position pays $21.00-$26.00 per hour depending on experience. Facilitate Individual Service Plan team meetings and develop a plan of care (ISP) for the client with input from the team. Plan will include a crisis plan when appropriate. In OHH, plans will integrate substance use, behavioral and physical health needs, and goals. Identify and assess physical and behavioral health related risk factors and ensure that clients are aware of all community services and resources. Assist clients in accessing resources. QUALIFICATIONS: Associates Degree required. Bachelor's degree preferred. MHRT/C or MHRT/C eligible. Spurwink Services is an Equal Opportunity Employer.
