Connected Home Health & Hospice, a Medicare-certified 5-Star Agency, is seeking an experienced and caring Home Health Occupational Therapist (OT). Mon-Fri schedule + weekends and Holidays off. * Independent scheduling allows for flexibility managing your own schedule.Duties: * Evaluates patient's functional status and home environment for hazards or barriers to more independent living. * Identifies equipment needs * Participates in the development of the total plan of care * Develops treatment program and establishes goals for improved function. Communicates plan of are to referring physician and other professionals * Teaches new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury * Fabricates splints and instruct patient in the use of various types of adaptive equipment to improve functionRequirements * Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Association and the American Occupational Therapy Association * Registered with the National Registration Examination of the American Occupational Therapy Association * BLS for Healthcare Workers from the American Heart Association (AHA) * Current Driver's license and auto insuranceBenefits include: * Medical/Dental/Vision * 401k Plan with company match * Health Savings & Flexible Spending Accounts * Life, Accident, & Disability Insurance * Mileage Reimbursement * Tuition Reimbursement * Long-term Service Recognition * Team Member Referral Program (up to $2,000) * Bonus & Productivity PerksConnected Home Health & Hospice is an Equal Opportunity Employer.
