Facility: Wellstar West Georgia Medical CenterThis position is a full-time role located at the Roosevelt Warm Springs location.Hours - 8:00am - 5:00pm Monday-FridayJob SummaryResponsible for assisting with the coordination of the daily operations for all preadmission activities, completing e Rehab and LTRAX assessments, participating in the planning, organizing, and executing of the business development plan, and developing marketing strategies through social media to expand the general public's knowledge of the facility. Essential responsibilities accomplished by working closely with the Nurse Navigators to convert referrals to admissions by providing responsive customer service and serving to promote RWSH.Core Responsibilities And Essential FunctionsProvides Marketing support by understanding healthcare operations, trends, and data analytics to promote and establish contact with potential customers or referral sources.Creates and provides education materials for referral sources, physicians on services, programs, and outcomes.Delivers content for social media accounts.Assists DBD with coordination of Admissions with the following tasks, but not limited to: maintaining tracking files, completing report emails, leading morning conference call, assisting Nurse Navigators and Admission Liaisons, providing facility tours, providing field coverage, collaborating with hospital directors and maintaining employee and patient confidentiality.Completes clinical evaluations by discussing with potential patients and families of pre-admission and admission processes.Accurately performs assessments and completes pre-admission screens in a timely manner.Facilitates and coordinates with physician and DBD as necessary for admission.Maintains compliance with Patient/Family-Centered care standards by providing information sharing, ensuring patients and families receive timely, complete and accurate information in order to effectively participate in care and decision-making.Maintains compliance with RWSH Customer Service standards.Other duties as assigned.Required Minimum EducationVocational or technical training or degree: Licensed Practical Nurse. Required.Required Minimum License(s) And Certification(s)Current American Heart Association, Basic Life Support. RequiredCurrent Licensed to practice as a nurse (LPN) in the State of Georgia or have an active Multi-State LPN Compact License. RequiredValid GA Driver's License. RequiredRequired Minimum Experience2 year(s) or more of experience in direct patient care as an LPN. Required1 year(s) or more of experience in navigating patients through the continuum of their care. PreferredRequired Minimum SkillsProfessional, organized, problem solving/critical thinking, communication skills - verbal and written. RequiredAbility to interact appropriately with patients from various backgrounds. RequiredMust be able to build and sustain trust with the physician, Clinical Manager and the interdisciplinary care team.
