GENERAL RESPONSIBILITIES AND DUTIES The Director of Care provides authority, responsibility, and accountability for the delivery of clinical services in the Company. In collaboration with Operations, helps to enable each resident to attain or maintain the highest practical physical, mental, and psychosocial well-being. Collaborates with other departments, medical professionals, consultants, and organizations, including government agencies and advocacy groups, to develop, support, and coordinate resident care, related administrative functions, and to represent the interests of Nazareth House and our values. ESSENTIAL RESPONSIBILITIES include the following. Other duties may be assigned Clinical oversight for assisted living services at the community Plans, coordinates, and evaluates Memory Care resident programs. Compiling and maintaining appropriate records and statistics. Monitors medication administration, medication records and medication documentation in the MARs for compliance issues. Maintains resident documentation, medical management documentation and care plan documentation. Reviews all records for areas of improvement. Reviews the care coordination of all residents with the Executive Directors and Nursing staff in each clinical area, as well as collaborating with physicians, employees, residents, visitors, and resident representatives in the development, implementations, and evaluations of performance improvement plans. Provides medical assessments of each new admission as well as ongoing resident assessments. Provides regular support to resident care needs and care-related follow-up as appropriate. Proactively develops positive employee relations, incentives, and recognition programs. Promotes teamwork, mutual respect, and effective communication. Monitors staff for compliance with OSHA mandates and corporate policies on workplace safety and safe work practices. Monitors the infection control program designed to provide a safe and sanitary environment designed to prevent the transmission of disease and infection. Ensure compliance with our COVID-19 plan. Coordinates and manages in-service training for staff on health conditions common to care services to include ADLs, medication management, safety, and regulatory compliance per CCLD. Communicates directly with residents, families, nursing staff, interdisciplinary members, and Executive Director to coordinate care and services, promote participation in-care plans, and maintain a high quality of care and life for residents. Continually seeks out and directs quality improvement activities that focus on wellness, clinical management, medication management, risk management, staff training, safety, and survey preparedness. Ensures compliance with all CCLD procedures, and reporting requirements as well as adherence to internal policies and procedures. Maintains knowledge of State Regulations and updates for CCLD. Maintains on-going communication with the Executive Director on all critical issues. Supports management and development of vendor relationships that are connected to clinical services at the community level. Maintains current knowledge of Alzheimer s/dementia related topics and participates in Memory Care training as assigned. Provides education, training, and hands-on support to community staff. Support the recruitment, hiring, and onboarding process of all licensed clinical personnel and care staff. Available for on-call duties on an as-needed basis. SUPERVISORY RESPONSIBILITIES This position does have direct supervisory responsibilities for all nursing staff. QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must have a minimum of 5 years of experience as a supervisor in a hospital, nursing care community or other related healthcare environment. Minimum of 3 years leadership experience in assisted living. Knowledge of CCLD regulations preferred. Must be a strong team player with proven leadership and supervisory skills. Consistently maintains high levels of resident satisfaction. Strong ability to communicate effectively with residents, families, staff, community officials, state regulatory agencies, hospitals, and the general public. Excellent customer service skills, with a courteous and helpful demeanor. Well-developed problem-solving skills and ability to develop conceptual alternatives Must have the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, procedures, etc., of the care services program. Willing to travel to other communities to assist with audits, training, and the setup of new facilities. CERTIFICATES, LICENSES, REGISTRATIONS Current and valid state certification as a licensed RN or LVN in the state of California. OTHER SKILLS AND ABILITIES Strong organizational and time management skills and the ability to prioritize responsibilities. Knowledge of current federal and state laws and regulations that apply to the practice of nursing in a health care setting. Strong customer service skills working with staff, families, and residents. Must be detail-oriented, have the ability to multi-task, and possess great interpersonal skills. Effectively and efficiently responding to identified quality concerns. Experience in navigating/managing the healthcare continuum: acute care, rehabilitation, skilled nursing, Medicare, Family Care functions, and specialty services. Management experience the ability to help support staff development, monitoring, training, performance improvement, accountability, etc. Excellent communication skills verbal and written. Understanding of all RCFE requirements by CCLD. Problem solving skills and ability to follow through. Flexible scheduling to accommodate training and scheduled on-call for emergent needs of residents. MINIMUM QUALIFICATIONS Knowledge of the requirements for providing care and supervision appropriate to the residents. Be in good health, and physically and mentally capable of performing assigned tasks. Good physical health and freedom from tuberculosis shall be verified by a health screening performed by a physician not more than (6) months prior to or (7) days after employment. Has current Adult First Aid card upon hire and maintains current status throughout employment. Must be criminally cleared by DOJ and FBI via the Live-Scan process prior to initial presence in the facility. Must have a clean driving record as per the insurance carrier’s policy. PHYSICAL REQUIREMENTS In a typical day, the Director of Nursing will routinely be required to stoop, carry, bend, squat, kneel, crouch, climb, grip, reach, drive, push and pull. The Director of Nursing will also occasionally be required to lift overhead, lift to waist high and lift to floor level. The Director of Nursing will also be required to use fine motor and manual dexterity skills in writing, keyboarding and grasping.
