DEPARTMENT: Nursing SUPERVISOR: Director of Nursing POSITION SUMMARY: The purpose of the LPN/LVN position is to participate in and oversee the provision of resident care as prescribed by the attending physician, consistent with accepted standards of care and within the scope of LPN/LVN training and licensure. LPN/LVN also provides direct resident care and assigns duties to subordinate nursing staff as appropriate. DUTIES AND RESPONSIBILITIES: The following represents the essential functions of the LPN/LVN's role. This list is intended to be a general description of duties and responsibilities; however, it is subject to change according to the needs of the facility determined by the Director of Nursing, Administrator, and/or corporate designee. Ensures facility residents receive high-quality, individualized care. Administers medications and treatments as prescribed. Assesses and reports resident changes in condition to the DON, Charge Nurse, attending physician, and responsible party as applicable. Documents change, notification of change, and new treatment orders/ plans. Explains treatment orders/ plans to residents and responsible party when needed. Receives, transcribes, and executes physicians' orders. Assists in collection of data relevant to resident treatment and assessments. Assures implementation of care plans as directed. Oversees/monitors tasks of subordinate staff. Evaluates staff performance and may initiate appropriate action. Answers phones and call lights. Keeps work area, medication carts, medication storage areas, and supply areas clean and organized. Provides and receives nursing report to maintain continuity of care. Investigates, documents, and initiates interventions as appropriate for resident incidents and accidents. May be required to investigate and document regarding staff or visitor incidents and accidents. Identifies, utilizes, and secures equipment and supplies. Delivers quality resident care consistent with accepted standards of care and established policy and procedure. Encourages safety to prevent accidents and assures equipment safety. Attends in-service training, Care Plan meetings, or any other meetings called by Legacy. Is responsible for using nursing judgement under the guidance of the Director of Nursing, RN Supervisor, Medical Director, attending physicians, nurse consultants, or other nursing professionals. Must act in ways that promote professional and positive representation of the facility to the community. Communicates in a professional and courteous manner using effective verbal and non-verbal communication skills with residents, their families, co-workers, and vendors. Maintains confidentiality of department, resident, employee information, communication, and documentation as well as compliance with the Health Insurance Portability and Accountability Act (HIPAA) regulations. Clocks in and out for all working/training hours. Working off the clock will not be permitted at any time or for any reason. Maintain compliance with Legacy's Compliance program and plan. Adhere to Legacy's Code of Conduct and Dress Code. These duties are not all-inclusive, but provide minimum performance expectations which will be reviewed in the evaluation process. Supervisor may edit, alter, add to, or exclude duties and responsibilities based on the needs of the facility. QUALIFICATIONS/EXPERIENCE/REQUIREMENTS: The requirements listed below are representative of the knowledge, skill, and/or ability required. Minimum Qualifications: Freedom from illegal use of drugs. Freedom from use and effects of drugs and alcohol in the workplace. Anyone found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care-related setting are ineligible for employment in the position. Education and/or Experience: Graduation from an accredited nursing school and a current, valid LPN/LVN license per the employing state's requirements. Experience in Long-Term care setting preferred but not required. Certificates, Licenses, Registrations: Current state license to practice as a nurse. Current CPR certification desired. LANGUAGE SKILLS: Ability to read, analyze and interpret technical procedures or governmental regulations. Ability to write reports, nursing/Resident progress notes, and nursing procedures. Ability to effectively present information and respond to questions from department heads, customers (Residents, family members, physicians, etc.), and the public. Ability to operate technology utilized by the facility including but not limited to PointClickCare, Microsoft Office Suite (Word, Excel, Teams, Outlook, etc.), iSolved, MCN (policy management), general computer operations, and other office equipment. REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form. RESIDENT RIGHTS: Promotes residents' rights, assists residents to make informed decisions; treats residents with dignity and respect. Reports suspected abuse or neglect in accordance with current professional standards and supports independent expression, choice, and decision-making. LEADERSHIP: Demonstrates willingness to try new tasks. Generates new ideas for change. Evaluates and recognizes priorities. Challenges others to learn. Keeps current and integrates new information. ENVIRONMENT OF CARE: Demonstrates understanding of fire and emergency procedures, participates in fire and disaster drills. Demonstrates understanding of safety and security procedures, applies safety and security precautions. Demonstrates understanding of hazardous materials plan and demonstrates proper use of equipment. INFECTION CONTROL: Applies hand washing principles during daily work. Demonstrates understanding of isolation precautions. Recognizes signs and symptoms of infection and complies with the employee health program. Demonstrates understanding of the process for identifying and handling infectious waste. Maintains personal hygiene. Complies with OSHA standards in the workplace and Demonstrate understanding of cross-contamination. INFORMATION MANAGEMENT: Records data as needed. Promotes confidentiality of resident information. Logs onto the system using own password. Logs off the system when leaving the workstation. Reports suspected violation of security/confidentiality issues. Reviews data and identifies trends. INTERPERSONAL SKILLS: Demonstrates active listening techniques and detail-oriented. Gains support through effective relationships. Treats others with dignity and respect. Thrives in a team setting and can direct the workforce. Seeks feedback. CONTINUING EDUCATION: Attends in-service and education programs and attends continuing education required for maintenance of professional certification or licensure. PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to stand, walk, use hands or fingers, reach with hands and arms, talk, or hear. The employee is occasionally required to sit, climb, or balance and stoop or kneel. The employee on occasion will assist to lift and/or transfer residents of varied weights and sizes. The employee will follow facility protocol on use of mechanical lifts and transfer devices. The employee will maintain coping skills regarding mental and emotional stress. WORK ENVIRONMENT: The work environment characteristics described are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee is occasionally exposed to blood or other body fluids, fumes, airborne particles, and toxic or caustic chemicals. The noise level in the work environment is usually moderate.
