Per Diem LVN Responsibilities and Duties for LVN Hospice: Perform skilled nursing tasks and home visits at patients' homes under the supervision of Registered Nurse Travel to patients' homes Prepare clinical notes, orders and charting in a timely manner Collaborate with physicians and agency staff to deliver the best care possible Work with patient and family in the home to achieve improvement in health goals Manage patients health care episode with the agency 1 Year experience in Hospice Reliable transportation Valid Driver's license LVN License On call duties after office hours (6pm) during weekdays and/or on weekends is optional This job description should not be interpreted as an exhaustive list of all responsibilities, duties, or skills that may be required of the employee.
