Position Title: Independent Living Services (ILS) CoordinatorEmployment Type: Part-time- Full-time (12-40 hours per week)Work Arrangement: Hybrid - Primarily conducted at individuals' homes or within the communityHours: Flexible, based on caseload demandsCaseload: 3-4 individualsCompensation:$21-22 an hr - based on experienceMileage reimbursement at $0.67 per mileRequirements: Current TB test within the last two years CPR and First Aid certification Ability to pass Live Scan background check Authorization to work in the United States Valid driver's license Access to a reliable vehicle Car insurance Minimum of 1-year experience working with individuals with developmental disabilities Responsibilities: Supervise and coordinate services for individuals served, ensuring their needs are met Accompany individuals to medical appointments Facilitate weekly planning sessions Develop comprehensive lesson plans with Specific, Measurable, Achievable, Relevant, and Time-bound (S.M.A.R.T.) goals Assist individuals in developing and managing life skills Provide transportation for individuals to community outings and appointments Join our team and play a vital role in empowering individuals to live independently and thrive within their communities!Powered by JazzHR
