Job Title: Home Health Registered Nurse RN Full TimeJob Summary:Aveanna Healthcare is seeking a skilled and compassionate Home Health Registered Nurse RN to provide high-quality patient care in the comfort of their own homes. As a Home Health Registered Nurse, you will be responsible for developing and implementing individualized care plans, educating patients and their families, and coordinating with healthcare teams to ensure seamless care.Responsibilities: Develop and implement individualized care plans in accordance with physician orders and patient needs. Provide education and training to patients and their families on disease management, medication administration, and other health-related topics. Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans. Coordinate with healthcare teams, including physicians, therapists, and other healthcare professionals, to ensure comprehensive care. Document patient progress and outcomes in accordance with regulatory requirements.Requirements: Active RN license in the state of application. Valid CPR certification. 1 year of experience in a healthcare setting, preferably in home health care.Preferred Qualifications: Medicare Skilled Nursing experience.
