Our team of licensed and certified professionals offer a wide range of in-home services, including Registered Nurses, Licensed Practical Nurses, Physical Therapists, Speech Therapy, Medical Social Workers, and Home Health Aides.Role DescriptionThis is a full-time role for a Physical Therapist. As a Home Health Physical Therapist, you will be responsible for coordinating the delivery of high-quality home health services for our patients. Your duties will involve facilitating timely and appropriate care, administering in-home therapy as needed, and developing customized care plans that meet the unique needs of each patient. You will also be responsible for monitoring patients' progress and adjusting care plans as needed to ensure optimal outcomes.QualificationsActive New Mexico Physical Therapist licenseMinimum of 1 year experience in home health or acute care settingExperience in physical therapy assessment and treatment planningAbility to assess and problem-solve patient care issues using a multidisciplinary approachDemonstrated ability to work effectively in a fast-paced, remote environment while maintaining a high standard of careStrong written and verbal communication skillsMaster?s degree in Physical TherapyExperience with Electronic Health Record (EHR) systemsCurrent CPR certificationValid Driver's License and reliable transportationPlease contact Moora or Monique at 505-293-XXXX for more information or to set up an interview.
