Job Title: Home Care NurseJob Summary:Aveanna Healthcare is seeking a skilled Home Care Nurse to provide high-quality patient care in the comfort of their own homes. As a Home Care Nurse, you will work closely with patients and their families to develop and implement personalized care plans that meet their unique needs and goals.Responsibilities: Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring. Conduct thorough assessments of patients' physical and emotional needs, and develop and implement individualized care plans. Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication. Administer medications and treatments as prescribed by physicians, and document all care provided in a timely and accurate manner. Provide education and support to patients and families on disease management, self-care techniques, and prevention strategies.Requirements: Current, unrestricted RN license in the state of practice. Minimum 6 months of recent experience as a Licensed Nurse in a clinical care setting. Home health experience preferred. Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
