A Brief Overview This role oversees the case management operations and provides supervision for the Family Advocacy and Support (FAS) Program. CLINICAL SUPERVISION Oversee the day-to-day outreach, assessment, and case management operations of the Family Advocacy and Support (FAS) Program including, GRIT assessments, intake reviews, documentation, and case management goals Oversee case management assignments Oversee and as needed, perform psychosocial assessments, therapy, and group interventions with clients and/or caregivers Share information regarding patient/caregiver situations with stakeholders including with staff from Healthier Together's GRIT Program, CHOP Primary Care, community agencies and programs Ensure compliance and incorporate standards and regulatory requirements into care Communicate program status to stakeholders and escalate and resolve potential issues Support the hiring and training, of new team members Supervise and develop staff including Community Based Social Workers and interns Performance MANAGEMENT Review data bases regularly and discuss barriers and solutions to meet goals Develop and utilize staffing strategies to meet client needs and the client care populations Evaluate staffing models to meet care needs and ensure work/case load distribution PROGRAMMATIC LEADERSHIP Develop and maintain relationships with patients, families, staff, leaders and physicians Promote and ensure sensitivity to cultural diversity through work and with teams. Identify barriers for families in accessing community services and advocate on their behalf. Identify resources to support client and family's recovery goals Collaborate with Healthier Together GRIT providers, Uplift and CGRC, to ensure appropriate and timely treatment for youth and families and to ensure streamlined processes Other duties as needed. Program Outreach Manage referrals and eligibility from various referral sources Conduct initial outreach to youth and caregiver to assess for needs and schedule assessment Complete consistent documentation in appropriate databases Education Qualifications Master's Degree Required Master's Degree Social Work Preferred Experience Qualifications At least one (1) year managerial or supervisory experience Required and At least five (5) years social work or healthcare related experience Required At least three (3) years managerial or supervisory experience Preferred or working with children and diverse populations.; Preferred or providing culturally sensitive counseling services to urban minority populations Preferred Skills and Abilities Ability to operate a company vehicle/be insured on a CHOP policy Basic proficiency with data management systems. Intermediate knowledge of trauma-informed care. Excellent verbal and written communications skills Excellent critical thinking / problem-solving skills Excellent time management skills Excellent organizational skills Excellent interpersonal skills Ability to maintain confidentiality and professionalism Ability to collaborate with stakeholders at all levels Licenses and Certifications Licensed Clinical Social Worker (LCSW) (Pennsylvania) Pennsylvania State Licensing Board Preferred Driver's License - Department of Motor Vehicle - upon hire - Required To carry out its mission, CHOP is committed to supporting the health of our patients, families, workforce, and global community.
