Community Nurse Job Description This company is seeking a skilled and experienced Community Nurse to work on a temporary agency contract within NHS Worcester. The successful candidate will be responsible for delivering evidence-based care to patients within their home environment who have complex needs. As a Community Nurse, you will work closely with specialist nurses, GPs, therapists and social work teams to provide collaborative support to patients and their families, and prevent avoidable hospital admissions.
