This is a Full-Time Temporary position. Job Summary: The CHW Program Supervisor will supervise a minimum of two Community Health Worker (CHWs) to help address social determinants and increase COVID-19 literacy with an emphasis in understanding public health measures and the importance of COVID-19 testing, vaccination, Respiratory Syncytial Virus (RSV) and Influenza in protecting families and communities in the designated counties assigned to this position. The CHW Program Supervisor must have shared life experiences in the communities they serve with shared values to support Community Health Workers as the intermediary link between health/social services and the community in order to facilitate access to services and improve the quality of service delivery.Key responsibilities: * Work with Local Health Department (LHD) to establish outreach plans and strategies to communicate information about COVID-19 and health/social issues to migrants/seasonal workers and immigrants in Cumberland County * Conduct phone and in person interviews and informational sessions with immigrant community members * Assist in conducting outreach to migrant/seasonal workers and immigrant communities, local churches and other organizations that serve the immigrant population * Help organize and host virtual and in-person community forums on COVID-19, social determinants of health and social drivers * Support the development and dissemination of outreach and informational materials within local communities and via social media platforms * Meet regularly with the CHWs to build understanding and social capital to support healthier behaviors and lifestyle choices * Document all interviews/information sharing contacts in project database to create an ongoing record of each case and provide detailed information for monthly metrics reports required by grantor utilizing the Unite Us Platform * Assist in the preparation of monthly narrative and statistical reports regarding outreach and client counseling or related efforts to be delivered to the funders and assigned local public health department * Organize and facilitate meetings with clients and providers in a variety of community locations and settings by linking them to resources including health insurance, food, quality care and health information, when applicable * Manage and efficiently lead a team and must be able to maintain a working knowledge of evolving community needs, and be able to deliver health information using culturally appropriate terms and concepts * Must be able to communicate with clients the purpose of the program, and have an understanding as well as knowledge of the local public health prevention measures in New JerseyRequirements: * Bilingual proficiency in English and Spanish is required * The CHW Program Supervisor must have shared life experiences in the communities they serve with shared values to support Community Health Workers as the intermediary link between health/social services and the community in order to facilitate access to services and improve the quality of service delivery * Understand the impact of cultural diversity, cultural humility, and cultural responsiveness training * Promote an environment of care, which is inclusive and respectful of every internal and external stakeholders to reduce health disparities and inequities * Effective problem-solving, organizational, and time management skills * Decision-making, priority setting, and work delegation abilities * Administrative/Technological skills, including Microsoft Office (Excel, Word, PowerPoint, Access), and Outlook * Excellent knowledge of available community services and resources * Ability to work independently or in group settings, meet strict deadlines, and manage or direct group activities * Ability to attend evening and weekend meetings * Strong communication and interpersonal skills * Comfortable with public speaking to diverse population * Maintain a high level of confidentiality * Excellent customer-focused interpersonal skills to interact effectively with health care practitioners, the interdisciplinary health care team, community agencies, patients, and families with diverse opinions, values, and cultural ideals * Must have a Valid NJ drivers' license and own an insured car; position requires some travel within the counties * Must be able to work legally in the United StatesEducation, Training and Experience * Associate degree required.
