Position Objective: Are you looking for a position to impact patients and families? Job Summary : As an On-Call RN of Kindful Health you will work with the Clinical Director to assist a care team including RNCM(s), LPN(s), Aides, MSW and Chaplain to provide best in class care to a reasonably sized case load. Essential Education and Experience : Must be a graduate of an approved school of professional nursing. Must have at least 2 years of nursing experience in the Hospice service line in the state of Texas. Responsibilities & Duties: RN will be responsible for: Adheres to all regulatory requirements and professional standards Follows policies and procedures consistent with recognized standards of professional practice Identifies system problems or inefficiencies that may negatively affect patient care Manage clinical excellence within your patient census. Provide high levels of communication both internally with the Kindful team and externally with patients and families to assure top CAHPS scores Collaborate with your Clinical Director, Medical Director and entire team to provide a "concierge" level hospice experience Qualifications & Skills : The best candidate for this opportunity demonstrates mastery of all facets of hospice care and displays a communication style that makes everyone who works with them want to do their best. This leader is just as happy in the field providing patient care as they are growing the knowledge and capabilities of their fellow team members.
